Grooming Release Request

Each customer is asked to sign a copy of this policy form at their first grooming appointment. If you have any questions please let the groomer know.. (Our currently groomer is not a stylish/ Professional license as she is a beginner.

I do hereby entrust my pet(s) to Atlas Pet Hospital for the purpose of grooming my pet(s). I am aware that all due care will be taken with my pet(s) for the safety and comfort of pet and groomer.

1) Veterinary Care: Your pet's comfort and safety is our priority. Atlas Pet Hospital will make every effort to ensure your pet's grooming is pleasant and safe. I authorize Atlas Pet Hospital to act as my agent in the event emergency veterinarian services, care-taking, and/or transportation is necessary for my pet and i agree to pay all expenses.

2) Safety Policy: Atlas Pet Hospital has the right to refuse services in the event a pet's behavior is compromising his/her safety or groomer. The use of muzzles, e-collars, etc are acceptable safety measures. Grooming services can be stopped mid-groom if necessary, for the safety of the pet and groomer. If your pet needs a chemical restrain, we will contact you to authorize us and please sign the sedation consent if needed

3) Matt Removal: Removing a heavily matted coat includes risks of nicks, cuts or abrasions due to warts, moles, or skin folds trapped in the mats. heavy matting can also trp moisture near the pet's skin allowing mold, fungus, or bacteria to grow, causing skin irritations that exist prior to the grooming process. After-effects of mat removal procedures can include itchiness, skin redness, self-inflicted irritations or abrasions, and failure of the hair to regrow. Shaved pets are also prone to sunburn and should either have sunscreen applied daily or should be kept out of the sun until the hair grows sufficiently to protect the skin. As the pet owner, i am responsible for the condition of my pet's coat and will not hold Atlas Pet Hospital responsible in the event of adverse effects of mat removal.

4) Flea/Tick Policy: If there is fleas and ticks, a fee will be charged. If the groomer sees a flea on your pet, a capstar will be administered according to weight of pet ~$13.50. fee will be added to my grooming bill. This treatment will rid the fleas for 24 hours, however this is not a permanent solution. Seek advice from your veterinarian for a monthly flea medication.

5) Photos/Pictures: Photos and pictures may be taken of my pet(s) before, during, or after grooming for use on Atlas/Babes dogs websites and advertising.

6) Vaccinations: Proof of vaccinations are required before the grooming session starts. A copy of vaccinations will be given to Atlas Pet Hospital to keep on file. Or I authorize Atals Pet Hospital to give my Pet the vaccines.

7) Signature required for grooming services: I understand and agree to the above terms for the grooming and maintenance of my pet(s) and in consideration of the grooming services of Atlas Pet Hospital to hold harmless from damage, loss, or claims arising from any known or unknown pre-existing condition of my pet(s). I have read and understand my rights and obligations as written in this agreement for the services of pet grooming through Atlas Pet Hospital .
