We make every effort to keep your pet in a sanitized, comfortable, safe and loving environment. However, some things do occur with greater frequency while pets are boarded things that can be attributed to being boarded are diarrhea and kennel cough Changes in diet, no matter how good the diet is, as well as the stress of being in a new environment (stress colitis) can cause diarrhea. Atlas Pet Hospital will use all reasonable precautions against illness, injury, or escape of my pet(s) but will not be held liable or responsible if any of these should occur while my pet(s) is boarding.If so, We recommend to have our veterinaria assess your pet and treat the pet accordingly. Pet has to be up to date in Bordetella, DA2PP, Rabies, If not, I Authorize Atlas Pet Hospital to do so.
Pet has to be up to date on Fleas/Ticks prevention and if not I authorize Atlas Pet Hospital to give my pet oral vs Topical dose>> Please choice
Should the circumstance arise that my pet remains unclaimed after pick-up date stated above, I understand that written notice will be mailed to the address above. Fourteen days after said written notice, my pet will be considered abandoned and I will no longer be the lawful owner of said pet. It is further understood that such action will not relieve me from responsibility of all costs related to the use of hospital and it’s services, including the cost of boarding services.
I do understand that there is no any body to observe my pet(s) overnight. after hospital hours
Please remove all items (leash, collar, tags) from your pet before leaving. We are not responsible for lost items left with your pet.
